Multifocal osteomyelitis in a preterm neonate: A case study

Osteomyelitis is a rare condition in the neonatal population with Staphylococcus aureus (S. aureus) the most common pathogen, followed by Klebsiella pneumoniae (K. pneumoniae). Diagnosis is difficult but important, due to the potential long-term sequelae of the infection. We discuss a case of a preterm neonate, born at 26 weeks’ gestation, who presented with S. aureus­­­­­­­­­ and evidence of osteomyelitis in the proximal humerus and femoral head. While rare, osteomyelitis is often multifocal. We explore the challenges of diagnosis, treatment and outcomes in this interesting case.

Where ethics and diagnoses collide – a case of congenital myasthenic syndrome

Neonatal congenital myasthenic syndrome (CMS) is a rare and life-threatening condition characterised by hypotonia and poor or absent respiratory effort; it may prove fatal. Managing this condition can be challenging due to difficult ventilation often needing high pressures, ventilator dependence and the lack of parallel planning involving palliative conversations.

Neonate with a pale, cold, pulseless arm: A case series

This is a case series of two neonates born in the UK with pale, pulseless, cold upper limbs that resolved with glyceryl trinitrate and heparin use. Both neonates had resolution of this presumed vasospasm and ischaemia prior to discharge from the neonatal unit.

 FREE  Non-pharmacological management in neonatal abstinence syndrome: an overview

Substance misuse continues to be a significant problem during pregnancy and is on the increase. Consequently, neonatologists are encountering an increasing number of infants born with symptoms of neonatal abstinence syndrome (NAS). Non-pharmacological measures are an effective means of managing infants with NAS and should be used in the first instance, however, most neonatal units do not have a guideline describing the non-pharmacological pathway, which is centred around promoting maternal involvement in care, minimising stimuli and increasing staff education. We outline the important aspects of non-pharmacological management of infants with NAS to encourage soothing strategies prior to the use of medications.
