Single centre experience of inhaled nitric oxide use in extremely preterm infants

This study reviews the use of inhaled nitric oxide (iNO) in infants born at less than 32 weeks’ gestation with hypoxic respiratory failure, over a 14-year period from 2010 to 2023. It describes trends in iNO utilisation, baseline characteristics of treated infants, and compares mortality outcomes with preterm infants who did not receive iNO. The article also examines the association between iNO use and timing of death. Implications for clinical decision-making in extremely preterm infants with hypoxic respiratory failure are discussed.

Congenital syphilis presenting at two months of age

Congenital syphilis remains a significant health burden globally and can have devastating long-term consequences if unrecognised and untreated, including deformities, hearing loss and premature death. We present a case of congenital syphilis diagnosed after two months of age in a previously healthy infant who developed multiple non-specific signs including rash, nasal snuffles and deranged blood markers. We discuss the importance of keeping rarer conditions such as congenital syphilis in mind during diagnosis, despite previously negative maternal serology.

The role of high flow therapy in optimising cardiorespiratory stability during intubation of premature infants

Premature infants frequently require endotracheal intubation, a procedure associated with significant risk of cardiorespiratory instability, including hypoxaemia, desaturation and bradycardia. High flow nasal cannula (HFNC) therapy has been proposed to optimise oxygenation and stability during intubation; however, its role in neonatal practice remains unclear. This systematised review evaluates the evidence for the use of HFNC during intubation of preterm infants, with a focus on cardiorespiratory stability. The findings suggest that HFNC may reduce episodes of desaturation and bradycardia during neonatal intubation, although further large scale studies are needed.

ROP screening: A question of how to manage infant pain

Retinopathy of prematurity (ROP) is a vasoproliferative condition affecting the retinal blood vessels of preterm and very low birthweight infants. It is a significant and preventable cause of visual impairment in childhood, so screening is crucial to prevent disease progression. Ophthalmologist examination is required, however, this can be uncomfortable and stressful as premature infants are highly sensitive to procedures. Studies show that inadequately managed pain can have significant consequences; it is important we monitor pain and implement ways to reduce it. This article examines current practice in the UK and the current literature.

FREE Are babies with congenital heart disease born in the right place at the right time? Practice in a cardiac network

Antenatal diagnosis of congenital heart disease (CHD) and subsequent delivery of higher risk cases in the tertiary centre improves perioperative conditions and neurodevelopmental outcomes in neonates with CHD. In October 2021, the Yorkshire and Humber Congenital Heart Disease Network developed a guideline with the objective of providing guidance on the timing and location of delivery for babies with antenatal diagnosis of CHD. Our project aimed to assess compliance with the guideline and evaluate the extent of improvement in delivery practices that have been implemented in accordance with the guideline.

Central Medical Supplies continues support for Ickle Pickles children’s charity

A partnership between the charity Ickle Pickles and healthcare equipment provider Central Medical Supplies, which has already raised nearly £6,000, wil continue for another year, with the aim of recruiting two peer support officers for neonatal units at Lewisham and Portsmouth hospitals.
